    How to make a referral

    1. Talk with the family — share what you've noticed and give them the tip sheet.
    2. Get parent consent on The Speech Source referral form — download it right here with the button below.
    3. Send us the form — email thespeechsource@gmail.com with the child's name, age, and concern.
    4. We take it from there — we contact the family within 1–2 business days and keep you in the loop with progress notes.

    This screener is an educational tool to help teachers decide when a referral may be helpful. It is not a diagnosis or a substitute for an evaluation by a certified speech-language pathologist.
